Fees: Florist License = $100.00 Per Year

Information Required with the Florist License Application:

1. Two passport size [2x2] photographs of the applicant.
   
2. Copy of valid driver's license, or other proof of identification.
   
3. Copy of applicant's social security card.
   
4. State Sales Tax Certificate of Authority
   
5. Proof of ownership or leasing (copy of deed/lease).
