  $110.40 First Pump Per Year
  $41.40 Each Additional Gas Pump

Information Required with the Public Gas Station Application:

1. Three passport size [2x2] photographs of the applicant.
   
2. Copy of valid driver's license, or other proof of identification.
   
3. Copy of applicant's social security card.
   
4. State Sales Tax Certificate of Authority.
   
5. Copy of lease or deed.
