Information Required with the Retail Food License Application:
- Two passport size [2x2] photographs of the applicant.
- Copy of valid driver's license, or other proof of identification.
- Copy of applicant's social security card.
- State Sales Tax Certificate of Authority.
- You must have zoning approval.
- Proof of ownership or leasing (copy of deed/lease).
- A Satisfactory Health Certificate ( if you are selling food products).
