Requests for proposals (RFP's) are on an as needed basis. They are advertised in the South Jersey Courier Post Newspaper, which is the official newspaper of the City. In the event of a discrepancy between anything on this website and what has been published in the Courier Post, the Courier Post publication shall prevail.

    • Business Entity Disclosure Certification (State Form)
     •  MC-4588 Business Entity Contribution Statement (City Form) Please note this form is required for Professional Services or Extraordinary Unspecifiable Services Contracts regardless of the $17,500 threshold noted above.

    •  MC-4588 Business Entity Contribution Statement (City Form) Please note this form is required regardless of the anticipated value of the contract.

-Required forms for Contracts for Professional Services or Extraordinary Unspecifiable Services awarded to non-profits
     •  MC-4588 Business Entity Contribution Statement (City Form) Please note this form is required regardless of the anticipated value of the contract.

Special Note: The City of Camden has adopted its own 'Pay- to- Play' Ordinance, MC- 4588, and related Business Entity Contribution Statement that must be completed by respondents to Requests for Proposals (RFP's) and all Professional Services Contracts and Contracts for Extraordinary Unspecifiable Services. Non-profits are not exempt from City Ordinance MC-4588 and are required to complete Business Entity Contribution Statements when responding to RFP's and are required for all Professional Services Contracts and Contracts for Extraordinary Unspecifiable Services. Anyone having questions relating to compliance with City Ordinance MC-4588 should contact the City Attorney at 856-757-7170.
